Terms of Service

These Terms and Conditions (“Terms”) apply to all professional health services (“Services”) provided by United Discount Chemists Pty Ltd ABN 50 116 361 776 trading as United Chemists (“we”, “us”, “our”). By accessing or using our Services, you (“you”, “your”, “patient”) agree to be bound by these Terms.

We provide a range of pharmacist-delivered health services, including but not limited to:

• Vaccinations and injection services (e.g., Influenza, COVID-19, whooping cough)
• Health screenings (e.g., blood pressure, blood glucose)
• Medication reviews
• Other clinical services in line with applicable professional standards and legislation.

Our health services are available to individuals who meet the relevant clinical and legal requirements.

• Patients 18 years and older may access services independently.
• Patients under 18 years of age may access certain services if permitted under state or territory legislation and if deemed clinically appropriate by the pharmacist.
• In some cases, a parent or legal guardian must be present.
• Where allowed by law, mature minors may be able to give their own consent, provided they demonstrate sufficient understanding (at the discretion of the pharmacist).

Limitations apply to certain services depending on your location. For example:

• Some states restrict pharmacist-administered vaccines to patients over a specific age
• Certain injectable medications may only be administered under a prescription or within a defined clinical scope
• Government-funded programs (e.g., NIP) may only apply to eligible age groups or risk categories.

You agree to provide accurate information about your age, medical history and current medications. If you are unsure about eligibility, please contact the pharmacy directly before booking.

• Bookings can be made online, in person or via phone.
• Walk-in availability is not guaranteed and depends on pharmacist or vaccine availability.
• Please notify us at least 24 hours in advance if you need to cancel/reschedule your appointment.

• Certain services may be government-funded (e.g., National Immunisation Program).
• Private services or non-eligible patients may incur an out-of-pocket fee.
• All fees will be clearly advised before the service is provided. Payment is required at the time of service unless otherwise arranged.

While our services are delivered by trained professionals using evidence-based practices, all health interventions may carry risk. You can ask your pharmacist about the risks relevant to your service. You will have the opportunity to discuss any concerns with the pharmacist prior to receiving a service. Our pharmacists are trained within their scope of practice in the event an adverse reaction should occur and emergency procedures are set in place.

It is your responsibility to inform the pharmacist of any known allergies or relevant medical conditions prior to treatment.

Your personal and health information is collected and handled in accordance with the Privacy Act 1988 (Cth) and relevant state/territory legislation to ensure your personal/sensitive information is protected.

Information may be shared with other healthcare providers (e.g., your GP, My Health Record) with your consent or as required by law.

You have the right to make informed decisions about your care. By participating in any service, you acknowledge that:

• You understand the purpose, benefits, risks and potential side effects
• You may ask questions or decline to proceed at any time
• You are providing voluntary and informed consent

Pharmacists delivering services will act within their professional and legal scope of practice. They may refer you to another health professional if your needs are beyond their scope.

These services are not a substitute for ongoing care by your GP or specialist.

While United Chemists take all reasonable care to ensure that health services are delivered safely and in accordance with professional and legal standards:

• You acknowledge that all medical and health-related services carry inherent risks and outcomes may vary between individuals.
• Our pharmacists will act within their professional scope of practice, but the information provided during services is not intended to replace advice from your doctor or specialist. Please consult your doctor or healthcare provider before making any changes to your medication, treatment plan or health management.

We are not liable for any injury, loss, damage or expense incurred as a result of: – Withholding or misrepresenting medical information

• Failure to follow aftercare instructions or medical advice
• Adverse reactions that are rare, unforeseeable or disclosed as possible risks

To the extent permitted by law, United Chemists, its employees and contractors disclaim all liability for any direct or indirect loss or damage arising from or in connection with the use of our health services. Nothing in these Terms excludes or limits your rights under the Australian Consumer Law or other applicable legislation

We may amend these Terms periodically. The latest version will always be available online at www.unitedchemists.net.au or can be requested to be printed in-store.

Terms and Conditions are governed by the laws of the state or territory in which the service is provided and the applicable Commonwealth legislation. By accessing our services, you agree to submit to the jurisdiction of the relevant courts and regulatory authorities in that state or territory.
